Please complete the online form below to request an AAA Permission to Skate Form, AA Non Resident Passport or a Junior Permission to Skate Form.
The completed PTS form will be emailed to you. Pick up will not be required.
The completed NRP from will need to be picked-up, once the form is ready, you'll receive an email to pick up.
MIDGET AAA PTS FORMS WILL NOT BE EMAILED OUT UNTIL AFTER AUGUST 23rd AS PER OHF RULES.
1. In order to receive a PTS or NRP form you will need to register and pay online "HERE". Returning players that have played AAA Hockey (that is recognized by the OHF) this past season will NOT be required to register or pay in full prior to receiving this form.
2. Registration and payment is NOT REQUIRED for junior player tryout permission forms.
3. Once payment is confirmed, the PTS form will be emailed to the email address provided in this form after April 4th. You will be emailed when the NRP form is ready and pick-up time will be made.
4. You are welcome to complete this form in advance of registering. However, please note, the completed PTS form will not be emailed until the registration is paid.
5. If a player that paid the registration fee is signed to an AAA team, a refund will be issued on request. Please send a refund request to the registrar at [email protected] .
